About The Position

The Non-Medical Case Manager is responsible for coordinating support services and assisting clients in accessing resources that improve health outcomes and retention in HIV care. The NMCM focuses on reducing barriers to care through referrals, linkage services, client education, advocacy, and coordination of non-medical support services. The NMCM assists clients with navigating systems of care while promoting self-management and engagement in treatment.

Requirements

  • A valid driver’s license and reliable transportation are required.
  • Ability to work within diverse populations.
  • Knowledge of Ryan White Part B services, as well as criteria in which clients must qualify for the program.
  • Knowledge of HIV care, treatment adherence, and community support resources.
  • Ability to maintain accurate and timely documentation.
  • Must be SOAR certified or receive certification within first 6 months of position.
  • The ability to gather relevant information about individuals to determine their needs and develop appropriate intervention plans.
  • The skill to support and empower clients by advocating for their rights, promoting self-determination, and fostering independence.
  • The ability to effectively communicate and build rapport with clients, colleagues, and other stakeholders.
  • The capacity to respond effectively to emergencies, crises or situations requiring immediate attention and support.
  • Ability to work independently and meet deadlines.

Responsibilities

  • Conducts psychosocial assessments and eligibility screenings.
  • Develops individualized service plans as required to determine and address barriers to better health and retention in care.
  • Facilitates referrals to outside agencies for supportive and non-medical services, including, but not limited to, housing assistance, food resources, transportation, counseling, insurance enrollment, financial assistance, and substance use services.
  • Provides linkage and re-engagement support for clients entering or returning to HIV medical care.
  • Performs case conferences with team members and community providers as needed to better serve client needs.
  • Provides periodic education regarding HIV transmission, risk reduction, insurance/enrollment, available community resources, and adherence support.
  • Provides client advocacy and assistance navigating healthcare and social service systems.
  • Maintains regular client contact to monitor progress toward goals and assess ongoing needs.
  • Maintains chart documentation in accordance with Ryan White Part B and agency standards.
  • Completes monthly compliance chart audits.
  • Attends RWB-mandated meetings or other required functions.
  • Performs other office/program duties as needed.
