We are thrilled to announce our recruitment for a Non-Medical Case Manager to join our highly experienced case management team. We are seeking an experienced individual to provide intense case management services to our clients who receive primary care at our clinic. Core services provided by candidate may include coordination of case with a multidisciplinary team to ensure treatment compliance and evaluating/anticipating ancillary, wrap-around and support services. The chosen candidate must be focused, able to handle multiple activities at once, have excellent written and oral communication skills, exceptional computer skills and be very detail oriented. Requirements

  • High School diploma or GED and 2 years of related experience or a certification as a Certified Community Health Worker (CHW)
  • Completed degree(s) from an accredited institution that are above the minimum requirements may be substituted on a year for year basis
  • Verification of education and licensure will be required if selected for hire
  • Possession of a valid unrestricted NM driver's license

Responsibilities

  • Coordinates patient documentation to maintain eligibility for a variety of core medical and social support services
  • Conducts patient interviews required to complete intake applications and all required forms for core medical and social support services
  • Works closely with medical case managers to reduce patient barriers to obtaining needed medical care and /or social services
  • Assist with the identification of community resources that build support for medically and socially complex patients
  • Maintain current information on all regularly accessed community resources
  • Perform a range of administrative functions required to ensure efficient program operation
  • Maintain professional growth and development through completion of required departmental and New Mexico Department of Health competencies
  • Perform miscellaneous job-related duties as assigned
