Provide non-clinical case management for eligible members with behavioral health and substance use disorders. Care is coordinated and/or integrated across complex healthcare systems, the patient's community, and internal systems. The role involves providing care facilitated by registries, information technology, and health information exchange to ensure members receive indicated care when and where needed, in a culturally and linguistically appropriate manner. This includes coordinating non-clinical resource referrals for housing, finance, food, shelters, and utility assistance, and referring crisis situations immediately. The position also involves attending events to develop community partnerships, providing behavioral health briefings, ensuring proper handling of consent forms, and preparing case management reports.
