About The Position

Nocturnist Nights are when a hospital’s safety net is tested. As nocturnist, you own the floors after the day team leaves—starting treatment on new admissions, catching deterioration before it becomes a crisis, and handing a clearer picture back at dawn. The role exists so patients still get a decisive physician when staffing is thinnest and families need a calm, capable presence.

Requirements

  • MD or DO and completion of an accredited internal medicine or family medicine residency.
  • Board certification, or active eligibility, in internal medicine or family medicine.
  • State medical license—or eligibility to obtain one before start—and DEA registration where inpatient prescribing requires it.
  • Current ACLS and a record of independent overnight admissions, cross-cover, and rapid-response leadership.
  • Privileges, or a defined path to them, for the procedures this night service expects.

Responsibilities

  • Admit overnight patients and start a working plan so the morning team inherits a diagnosis, orders, and a documented rationale—not a backlog.
  • Cover the assigned census, triaging nurse concerns and intervening early enough to cut avoidable ICU transfers and delayed escalations.
  • Lead rapid responses and codes on your units until the patient is stable or the right next team has assumed care.
  • Finish notes and medication reconciliation before sign-out so day clinicians can act without reconstructing the night.
  • Hand off only what changes management, closing the loop with specialists and the incoming hospitalist.
  • Perform credentialed bedside procedures promptly, or set a clear alternative plan when a procedure is outside your privileges.
