This role involves two primary functions: Rehabilitation Specialist and NMT Driver. As a Rehabilitation Specialist (8:30 am - 2:00 pm), you will provide services to adults in adult day services or vocational preparation programming. This includes developing activities to keep individuals active and engaged, teaching work skills, assisting with workflow for continuous work opportunities, and helping individuals increase daily living skills and personal/social adjustment. You will shape personal and social behaviors through coaching, modeling, and demonstration. You will maintain a caseload of up to 12 individuals, including case notes, ISP revision dates, documentation, recommendations for transition, and keeping supervision updated on individual progress. Professional communication and a positive work atmosphere are essential. As an NMT Driver (2:00 pm - 3:30 pm), you will safely transport individuals with developmental disabilities to and from day services using a non-CDL vehicle. Responsibilities include completing daily trip checks, noting service needs, assisting passengers (including securing wheelchairs), following designated routes, reporting emergencies or disruptions, maintaining customer service, performing bi-weekly vehicle cleaning, managing vehicle servicing (fuel, lubricants), removing trash, maximizing gasoline usage, parking and securing the vehicle, recording mileage and destination information, maintaining vehicle documentation, and adhering to Medicaid rules and CARF standards. You will also complete required paperwork for Medicaid billing and submit transportation sheets, and immediately report any unusual behavior or vehicle issues, following mandatory reporting guidelines.
