This role combines two functions: Rehabilitation Specialist and NMT Driver. The Rehabilitation Specialist provides services to adults in day services or vocational preparation programming, developing activities to keep individuals active and engaged. If in vocational preparation, the specialist teaches work skills and assists with workflow to provide continuous work opportunities. The specialist also assists individuals in engaging in programming designed to increase daily living skills and personal and social adjustment, shaping behaviors through coaching, modeling, and demonstration. This role includes maintaining a caseload of up to 12 individuals, managing case notes, ISP revision dates, documentation, recommendations for transition, and keeping supervision updated on individual progress. The NMT Driver safely transports individuals with developmental disabilities to and from day services using a non-CDL rated vehicle. Responsibilities include completing daily trip checks, noting service needs, assisting passengers on and off the vehicle, securing wheelchairs, following designated routes, reporting emergencies or disruptions, maintaining customer service, performing bi-weekly vehicle cleaning, managing vehicle servicing (fuel, lubricants, accessories), removing trash and sweeping the vehicle daily, maximizing gasoline usage, parking and securing the vehicle, recording mileage and destination information, maintaining insurance, safety, and vehicle maintenance information, following Medicaid rules and CARF standards, completing required paperwork for Medicaid billing, and reporting unusual behavior of passengers or vehicle issues while following mandatory reporting guidelines.
