About The Position

The Night Hospice Supervisor serves as a resource for Hospice patients after normal business hours, handling all nighttime calls from patients/families, other healthcare providers, and vendors. This role requires exercising discretion and independent judgment regarding significant matters related to the agency's operations and patient care. The supervisor will also conduct patient visits during the night when necessary, operating on a 7 On/7 Off rotation. The pay rate for this position is between $85k and $94k.

Requirements

  • Current and unrestricted Idaho Licensure or Compact State Licensure
  • Nurse: 1 year (Required)
  • Ability to function well in a high-paced environment.
  • Ability to maintain confidentiality, and to exercise discretion and good judgement.
  • Complies with company guidelines/policies, as well as governing state and federal laws and regulations.

Responsibilities

  • Providing skilled nursing, intervention, and supportive care to patients in the home setting for after regular business hours in accordance with the Company’s Plan of Care for each patient.
  • Carrying out the plan of care.
  • Assisting with the preparation, implementation, and continuing evaluation of the patient plan of care.
  • Actively participating with the IDG in collaborating to ensure the patient’s plan of care is kept current and any identified issues are addressed.
  • Reinforcing teaching to the patient/family members regarding end of life care.
  • Fostering cooperative effort among personnel by understanding the functions of other persons involved in patient care and by active participation in team and staff conferences.
  • Assuming responsibility for personal and vocational growth and development through active participation in nursing organizations and participating in in-service programs and other on the job learning.
  • Preparing clinical and progress notes as appropriate.
  • Keeping the Case Manager informed of changes noted during visits.
  • Answering all phone calls received on behalf of the agency and following up according to agency policies and procedures.
  • Educating primary caregivers, volunteers, and employed caregivers to provide care for patients as indicated.
  • Maintaining regular communication with agency staff and the patient’s referring physician and/or case manager concerning patient/family care needs and response to interventions.
  • Maintaining up-to-date patient records so that problems, goals, and interventions are accurately and clearly stated, and changes are reflected as they occur.
  • Completing documentation within agency guidelines and to meet regulatory requirements.
  • Submitting completed documentation within agency guidelines and to meet regulatory requirements.
  • Communicating regularly with agency nursing staff to review problems or unique issues from on-call hours.
  • Performing other tasks as assigned.
