About The Position

Transitional Services for New York, Inc. (TSINY), a not-for-profit, comprehensive, community-based mental health organization, is seeking a Full-time Night Counselor for one of its Residential Programs in the Bronx, NY. TSINY provides a continuum of rehabilitative services to enrich the lives of individuals recovering from mental illness and to facilitate their transition to increased independence. The organization aims to broaden its rehabilitative services and become a regional social service provider, delivering effective programs with compassion, integrity, and professionalism, prioritizing client needs while respecting staff and fostering hope.

Requirements

  • High School diploma required.
  • NYS Driver’s License with a clean record required.
  • Driver’s license required.

Responsibilities

  • Provide oversight of the program on the overnight shift.
  • Complete room checks and determine that all residents are accounted for.
  • Conduct chart audits and maintain resident back charts.
  • Provide crisis management during overnight hours.
  • Get residents up in the morning and ready for daytime activities.
  • Converse with the shift going off duty for pertinent information.
  • Inform residents of medical/psychiatric or financial appointments.
  • Supervise morning medication and initial the medication book.
  • Follow procedures according to the change of shift checklist.
  • Ensure residents' beds are made before exiting the facility.
  • Record staff absence and/or lateness in the Call-in Book if applicable.
  • Check staff revised schedules and email.
  • Check personal mailbox.
  • Intervene in resident crisis management.
  • Assist residents with meal preparation when needed.
  • Report pertinent information regarding residents and facility to the shift supervisor.
  • Document pertinent information regarding residents in the Communication Book.
  • Follow procedure for change of shift checklist with the on-coming shift.
  • Monitor residents' bathing, showering, and hygiene as needed.
  • File all chart items into the appropriate charts.
  • Facilitate back charting.
  • Review Medication Book for initials.
  • Meet with supervisor as scheduled.
  • Participate in and supervise residents during fire drills.
  • Inspect residents' rooms for safety measures and report damages or problems.
  • Conduct monthly chart review as assigned by supervisor.
  • Attend CPR/First Aid training.
  • Make copies of new forms and dispose of old ones.
  • Call in refills of medications.
  • Prepare information for shredding.
  • Conduct chart audits.
  • Perform regular building tours to check for cleanliness and needed repairs.
  • Check on all residents during building tours.
  • Complete work orders.
  • Complete incident reports on the same day of the incident and give the report to the shift supervisor or administrators.
  • Submit receipts for petty cash spent.
  • Attend special on-site and off-site training, seminars, conferences, etc.
  • Be available to residents on an as-needed and crisis basis.
  • Submit requests for time off at least two weeks in advance.
  • Check revised schedules for possible changes in hours/shifts and for holiday coverage.
  • Return correspondence that states, "Sign and return."
  • Familiarize with personnel policy, emergency manuals, and training manual.
  • Provide supervision for residents during dining hours.
  • Ensure residents are dressed appropriately before leaving the facility.
  • Perform other related duties as required.
  • During a declared disaster, assume and adhere to assigned Job Action role(s) consistent with Disaster Preparedness Plan.
