Responsibilities
- Provide direct patient care to our residents, including personal care, vital sign monitoring, and assistance with activities of daily living
- Work closely with our nursing team to ensure seamless communication and coordination of care
- Participate in resident assessments, care planning, and implementation of individualized care plans
- Maintain accurate and up-to-date records of patient care and treatment
