About The Position

Infinite Medical P.C. is a nationwide network of advanced practice providers and specialty clinicians delivering proactive care in skilled nursing and long-term care facilities. In partnership with MedElite Healthcare Management Group, the organization focuses on compassionate, personalized care for residents. The Neurology Nurse Practitioner/Physician Assistant provides comprehensive care to patients with neurological disorders, including epilepsy, multiple sclerosis, and Parkinson’s disease, while working with a dedicated team of healthcare professionals. The position is part-time and located in Cleveland, Texas.

Requirements

At least 6 months of experience in long-term care or a skilled nursing facility.
Active and unrestricted Nurse Practitioner, Physician Assistant, or Physician license.
DEA license.
CPR BCLS/BLS certification.
Master’s degree in Nursing.
Degree from an accredited Nurse Practitioner program.
One year of experience in Neurology.
Great organizational and time-management skills.

Nice To Haves

Experience documenting in an EMR such as Sigma, Visual, or PCC; training is provided.

Responsibilities

Develop and implement individualized treatment plans, including medication management and non-pharmacologic interventions.
Conduct regular follow-up visits to monitor patient progress, adjust treatment plans, and provide education and support.
Collaborate with physicians and other healthcare professionals to coordinate care and support a multidisciplinary approach.
Educate patients and families about disease management, medications, and lifestyle modifications.
Maintain accurate and timely documentation of patient encounters and treatment plans.
Participate in quality improvement initiatives to enhance patient outcomes and optimize care delivery.
Perform comprehensive assessments to diagnose and treat patients with neurological conditions.
