About The Position
We are seeking an experienced Nurse Practitioner or Physician Assistant to join a growing nephrology team. The provider will work closely with the physicians and clinical team to care for patients with acute and chronic kidney conditions in an outpatient setting.
Requirements
- AZ Licensed Nurse Practitioner or Physician Assistant
- National board certification
- Nephrology or dialysis experience required
- Strong clinical, communication and patient-care skills
Responsibilities
- Evaluate and manage patients with chronic kidney disease, hypertension, electrolyte disorders and other renal conditions
- Provide follow-up care for dialysis patients
- Perform patient assessments, develop treatment plans and order appropriate diagnostic testing
- Review laboratory results and adjust medications as needed
- Educate patients and families about kidney disease, medications and treatment options
- Collaborate with physicians, dialysis centers and other members of the healthcare team
- Maintain accurate and timely medical documentation
