About The Position

The Journey Navigator RN's primary function is to seamlessly guide the patient and family through the health care system. Activities include providing disease-specific education and information, referral navigation, sharing knowledge in regard to treatment options, expediting patient access to diagnostic and treatment procedures, connecting patients with appropriate resources, providing ongoing support to patient and family, responding to patient/family needs and addressing issues of access to care as part of the navigation team. Collaborates with social work, patient financial counselor, chaplains, dietitian, primary care, cardiac rehab, heart team and volunteers to provide comprehensive navigation services to patients, families and the community at large. Requirements

  • Graduate of an accredited school of nursing.
  • Associate's Degree in Nursing
  • Montana Registered Nurse License
  • National Provider BLS - American Heart Association
  • 3 years professional nursing experience
  • 1 year nurse navigation or similar experience

Nice To Haves

  • Bachelor’s Degree in Nursing
  • Related experience in an ambulatory setting, preferably in a medical office
