About The Position

Serves as a patient advocate, liaison, and adviser/educator to assist patients with navigating the continuum of care. This role is crucial in ensuring patients and their families receive comprehensive support throughout their healthcare journey. The Navigator acts as a bridge between patients, clinical staff, administration, physicians, and external resources to coordinate care and overcome barriers.

  • BLS American Heart E-Card (BLS AHA ECARD) - American Heart Association (AMERICAN HEART), BLS American Heart RQI E-Card (BLS AHA-RQI ECARD) - American Heart Association (AMERICAN HEART), BLS Cert American Heart_non ecard (BLS AHA) - American Heart Association (AMERICAN HEART), BLS Certification Grace (BLS GRACE) - Employee Grace Period for Essential Credential (GRACE), BLS Cert Red Cross (BLS RC) - Red Cross (RED CROSS)
  • Registered Nurse Compact License (RN LICENSE COMPACT) - Compact RN Multistate, Registered Nurse License (RN LICENSE) - State of Florida (FL)
  • One (1) year of experience in related field.
  • If RN, must be licensed.
  • Graduate of an accredited registered/Professional Nursing Program or MSW degree.
  • Radiation Therapy license required if working in the Radiation Oncology department.

Nice To Haves

  • Master's in Social Work (MSW) degree preferred if degreed.
  • (RT) and certified in Radiation Therapy (T) by the American Registry of Radiologic Technologists (ARRT) - Radiation Therapy License required if working in Radiation Oncology.

Responsibilities

  • Serves as a liaison between patient and clinical staff, administration, physicians, and community/external resources to coordinate/maximize resources and identify/resolve barriers to the plan of care.
  • Assists patients and families with resolving financial, psychosocial, functional, and administrative issues by advising of options and referring to appropriate resources.
  • Intervenes as appropriate to advocate for patient and family.
  • May collect and assess data to identify quality trends or actual sources of risks to patients and employees.
  • Collaborates with multidisciplinary team and committees with respect to quality outcomes.
  • Continuously reviews and evaluates patient progress as outlined within the plan of care.
  • Communicates plan of care to patient and family and solicits concerns, questions, and issues for resolution.
  • Provides emotional support, counseling, clinical education and expert guidance to patients and families to promote their ability to understand and meaningfully participate in the healthcare process and personal decision-making.
