Requirements

  • Active, unrestricted Registered Nurse (RN) license in California
  • Eligible to serve as Administrator for a California Home Health Agency (meets Title 22 requirements)
  • Minimum 7–10 years of progressive leadership experience in home health, hospice, or related healthcare services
  • Demonstrated experience in building or scaling healthcare programs, service lines, or new markets
  • Strong knowledge of California home health regulations and compliance requirements
  • Proven ability to lead both clinical and operational teams

Nice To Haves

  • Experience in private-pay or concierge care models and home health
  • Direct experience launching de novo healthcare operations
  • Familiarity with nurse registry models and multi-state regulatory environments
  • Experience with accreditation bodies (e.g., CHAP, ACHC, Joint Commission)

Responsibilities

  • Lead the build-out, launch, and scale of private-pay home health services in Southern California, with planned expansion into Northern California
  • Act as the architect of the service line, designing care models, workflows, and operational infrastructure from the ground up
  • Serve as Administrator of Record for the Southern California home health agency, ensuring compliance with all state licensing and regulatory requirements
  • Partner with the President to define and execute market entry strategy, growth roadmap, and long-term vision
  • Build foundational elements including policies, SOPs, staffing models, and clinical programs aligned with a concierge standard of care
  • Drive patient census growth while maintaining premium service quality and clinical outcomes
  • Provide strategic and operational oversight of the Concierge Nursing Referral Services (CNRS) across all active states
  • Evolve and scale the nurse registry model to align with the broader clinical services growth strategy
  • Standardize and build upon best practices, quality expectations, and service delivery models across markets
  • Partner with regional leaders to drive 1099 clinician engagement, retention, and performance
  • Ensure compliance with state-specific nurse registry regulations and mitigate risk
  • Establish and build clinical quality frameworks and performance standards across both home health and registry service lines
  • Stand up QA/PI programs, documentation standards, and audit processes from inception
  • Partner with DPCS and clinical leaders to ensure high-quality patient outcomes, documentation integrity, and survey readiness
  • Lead and personally support state survey readiness and onsite survey activity, including interfacing directly with surveyors as Administrator
  • Drive readiness for state surveys, audits, and potential accreditation processes (e.g., CHAP)
  • Recruit and build a high-performing interdisciplinary team (RNs, LVNs, therapists, and operational leaders as the model evolves)
  • Develop leadership structure and create scalable org design to support growth
  • Foster a culture aligned with company values, emphasizing accountability, collaboration, and excellence
  • Partner cross-functionally with sales, marketing, and operations to drive referrals and revenue growth
  • Support development of strategic partnerships (e.g., concierge medicine, private physicians, high-net-worth client channels)
  • Build scalable infrastructure through technology selection, EMR optimization, and process design
  • Monitor financial performance and contribute to budgeting, forecasting, and P&L management
