As the Multi-Service Navigator , you will play a key role in advancing Cardinal McCloskey Community Services' vision of operating as a fully integrated multi-service organization. This position is responsible for connecting children, adults, and families to the broad continuum of CMCS programs and services through holistic assessment, client engagement, and strategic cross-program referrals.

Responsibilities

  • Manage a caseload of children in foster care, ensuring their safety and well-being
  • Develop and implement individualized treatment plans focused on permanency and stability
  • Support foster parents with parenting strategies and behavioral guidance
  • Maintain consistent communication with families, schools, and service providers
  • Represent cases in Family Court and ensure every child's voice is heard
  • Document progress and maintain compliance with agency and ACS standards
  • Participate in intake activities to assist treatment teams with referral recommendations.
  • Facilitate all internal referrals and collaborate with the designated program point person.
  • Attend regular treatment meetings to assist in identifying resources for the client/ family.
  • Serve as a resource to Staff for identifying appropriate internal and external programming.
  • Monitor and track cross divisional and external referrals, ensuring closed-loop communication with the treatment team via the Electronic Health System.
  • Maintain accurate and timely documentation in the Electronic Health Record (EHR).
  • Identify and develop partnerships with other community-based service providers.
  • Train Staff across the organization on full continuum of CMCS services and the Multi-service Approach.
  • Support data collection and tracking in CMCS' multiservice dashboard, playing an active role in reviewing, interpreting and enhancing data in partnership with Program and QI leadership.
