This position is responsible for completing psychosocial assessments for patients' post-hospital care, assisting patients in coping with stress related to illness, and collaborating with the Clinical Care Management team to determine appropriate care settings. The role also involves serving as a resource to hospital staff and physicians on psychosocial aspects of patient care, providing information and education on community resources, making referrals to community agencies, and reporting suspected cases of abuse/neglect/exploitation. The Case Manager also acts as a liaison between the hospital, patients, and the Department of Social Services, and responds to consultative requests after normal working hours.
