About The Position

The MSW Case Manager completes psychosocial assessments for patients' post-hospital care, assisting them and their support systems in coping with stress related to hospitalization, disability, chronic/terminal illness. They work with the Clinical Care Management team to assess and evaluate patients for appropriate levels of care or environment settings across the continuum. The role serves as a resource to hospital staff and physicians regarding emotional, social, and psychosocial components of illness and its effects on social support systems. In collaboration with the Clinical Care Management team, the Case Manager provides information and education on community resources and post-hospital care options, making referrals as needed. They are also responsible for reporting suspected cases of child and adult abuse/neglect/exploitation and serving as a liaison between the hospital, patients, and the Department of Social Services during evaluations/investigations. The position also involves responding to requests for consultative services after normal working hours.

Requirements

  • Masters in Social Work
  • Adherence to National Association of Social Workers Code of Ethics

Responsibilities
