About The Position

This position involves conducting Michelle P. Waiver (MPW) assessments and reassessments. This includes face-to-face visits with individuals in their homes within state-defined deadlines. The role requires completing initial assessments for new individuals to the MPW program, providing information on service delivery, and guiding individuals to case management agencies. A key responsibility is documenting responses using a fifteen-page state-designated assessment tool that covers health, daily living activities, neurological and emotional status, social supports, and physical environment. The position also involves obtaining consents, completing agency and state forms, and securely sharing documents with individuals and medical records staff. Additionally, the evaluator will complete electronic medical records entries and submit MPW evaluations to the Quality Improvement Organization for Level-of-Care approval. Communication with individuals, families, and case managers regarding assessment status is essential.

Requirements

  • Registered nurse or social worker (active and unencumbered RN license, or Bachelor’s degree in social work, sociology, psychology or a related field as approved by Medicaid).
  • One year of experience with the DD/ID population with a Bachelor's degree, or six months of experience with the population with a Master's degree.
  • Working knowledge of the MPW and eligibility criteria.
  • Reliable transportation for frequent travel.

Responsibilities

  • Conducts Michelle P. Waiver assessments and reassessments, including face-to-face visits within the 14-day deadline or other state-established deadlines.
  • Completes initial assessments for individuals new to the Michelle P. Waiver.
  • Provides information on service delivery types and directs individuals to available case management agencies.
  • Documents individual responses using a fifteen-page assessment tool covering health, daily living activities, neurological and emotional status, social supports, and physical environment.
  • Obtains consents and completes agency and state-assigned forms.
  • Ensures documents are shared securely with the individual and medical records staff.
  • Completes information notes and assigned documents in the electronic medical records system within given deadlines.
  • Arranges for submission of the MPW evaluation to the Quality Improvement Organization for Level-of-Care approval.
  • Communicates with individuals, families, and case managers on the status of their assessment throughout the evaluation process.
