Mobile Prevention Coordinator
About The Position
Easterseals PORT Health (ESPH) is seeking a compassionate and dedicated full-time Mobile Prevention Coordinator to join their team in Northern Virginia. The role involves providing clinical and coordination services for the REACH Mobile Prevention Team in individuals' homes and community settings. Primary duties include on-site and telephonic/telehealth crisis prevention and intervention services (crisis planning), safety planning, and education. This position is responsible for a shared rotation of a 24/7/365 crisis line, which includes on-site response. The role requires local travel within Arlington, Alexandria, Fairfax, Loudon, and Prince William counties.
Requirements
- Bachelor’s degree in human service field
- QMHP-T or eligible to be a QMHP-T within 30 days of hire
- Valid driver's license, good driving record and current auto insurance
- Proficiency with EHR systems or technically savvy
Nice To Haves
- 3 years of experience working with individuals with developmental disabilities and mental health needs
- Strong crisis intervention skills preferred
Responsibilities
- Provide clinical and coordination services for the REACH Mobile Prevention Team in the individuals home and/or community settings.
- Conduct on-site and telephonic/telehealth crisis prevention and intervention services (crisis planning).
- Perform safety planning and education.
- Share rotation of a 24/7/365 crisis line, including on-site response.
- Travel locally to Crisis Therapeutic Home (CTH) and the community.
