About The Position

Easterseals PORT Health (ESPH) is seeking a compassionate and dedicated full-time Mobile Prevention Coordinator to join their team in Northern Virginia. This role provides clinical and coordination services for the REACH Mobile Prevention Team, offering on-site and telephonic/telehealth crisis prevention and intervention services, safety planning, and education in individuals' homes and community settings. The position involves a shared rotation of a 24/7/365 crisis line, including on-site response, and local travel within Arlington, Alexandria, Fairfax, Loudon, and Prince William counties. Requirements

  • Bachelor’s degree in a human service field.
  • QMHP-T or eligible to be a QMHP-T within 30 days of hire.
  • 3 years of experience working with individuals with developmental disabilities and mental health needs.
  • Strong crisis intervention skills preferred.
  • Valid driver's license, good driving record, and current auto insurance.
  • Proficiency with EHR systems or technically savvy.
  • Must reside or be willing to relocate to one of the listed service areas (Arlington, Alexandria, Fairfax, Loudon, and Prince William).

Responsibilities

  • Provide clinical and coordination services for the REACH Mobile Prevention Team in individuals' homes and/or community settings.
  • Conduct on-site and telephonic/telehealth crisis prevention and intervention services (crisis planning).
  • Perform safety planning and education.
  • Share rotation of a 24/7/365 crisis line, including on-site response.
  • Travel locally to Crisis Therapeutic Home (CTH) and the community.

Benefits
