About The Position

Stepping Stone Behavioral Health is seeking dedicated and compassionate Licensed Social Workers (LSW/LISW), Licensed Professional Counselors (LPC/LPCC), or Licensed Chemical Dependency Counselors (LCDC II, LCDC III, LICDC) to join their outpatient behavioral health team. This mobile clinician role provides mental health and substance use counseling directly inside skilled nursing and long-term care facilities, offering meaningful support to residents coping with emotional, behavioral, and substance-related challenges. Requirements

  • Must be 21 years or older with a valid driver’s license.
  • Active Ohio license: LSW, LISW, LPC, LPCC, LCDC II, LCDC III, or LICDC.
  • Ohio Medicaid Billing Number and NPI Number required (or ability to obtain).
  • Minimum of an Associate’s Degree (Bachelor’s or Master’s preferred).
  • Strong interpersonal, organizational, and documentation skills.

Responsibilities

  • Conduct bio-psychosocial assessments and develop individualized treatment plans.
  • Provide individual and group counseling, teaching coping and recovery skills.
  • Identify resident needs, set measurable goals, and guide them toward emotional stability.
  • Participate in treatment team meetings and collaborate with facility staff and community partners.
  • Maintain timely and accurate clinical documentation in EMR systems.
  • Assist with telehealth sessions and submit required daily reports.
  • Facilitate safe discharge planning and coordinate aftercare services.
  • Uphold confidentiality, professionalism, and compliance with facility and state regulations.
