About The Position

Improve the overall health of the communities we serve by enhancing access to quality health, wellness, and supportive services. This role involves performing independent visits outside of the clinic, providing various health services, and collaborating with multiple teams to support population health and health equity initiatives. The position also focuses on care coordination, case management for unengaged patients, and patient/family education, addressing social determinants of health.

Requirements

  • Valid Washington State RN License.
  • Excellent customer service, oral and written communications skills required.
  • Compassion for patients required.
  • Must be able to work well in a team environment.
  • Ability to multi-task in fast-paced work environment, with ability to anticipate future needs required.
  • Computer skills required.
  • Basic technology troubleshooting skills required.
  • BLS (CPR/AED) required.
  • Commitment to supporting a safe and respectful environment is required.

Nice To Haves

  • Bachelor of Science degree in Nursing from four-year college or university, preferred.
  • Minimum of three years’ experience in the clinic setting as a Registered Nurse preferred.
  • Experience in triage and experience in the Community Health setting a plus.
  • Experience with electronic health record strongly preferred.
  • Experience with process improvement a plus.

Responsibilities

  • Performs independent visits while operating under standing order procedures for patients outside of the clinic. This may include but is not limited to providing health assessments, medication reconciliation, wound care, lab work, immunizations, and patient education.
  • Works collaboratively with mobile medicine, outreach, and primary care teams to support population health initiatives.
  • Supports health equity team in identifying health outcome initiatives to reduce health disparities for marginalized and excluded patient populations.
  • Leads efforts to coordinate care and perform case management with unengaged patients to improve their health outcomes through creating care plans to improve chronic conditions and achieve more effective personal, social, education and vocational development.
  • Performs patient, family and caregiver education, including motivational interviewing and patient centered health action plans.
  • Collaborates with other resources within organization and in the community to address patient’s social determinant of health needs.
  • Performs other duties as assigned, including supporting the CHAS Health Mission and Core Values.
