Improve the overall health of the communities we serve by enhancing access to quality health, wellness, and supportive services. Performs independent visits while operating under standing order procedures for patients outside of the clinic. This may include but is not limited to providing health assessments, medication reconciliation, wound care, lab work, immunizations, and patient education. Works collaboratively with mobile medicine, outreach, and primary care teams to support population health initiatives. Supports health equity team in identifying health outcome initiatives to reduce health disparities for marginalized and excluded patient populations. Leads efforts to coordinate care and perform case management with unengaged patients to improve their health outcomes through creating care plans to improve chronic conditions and achieve more effective personal, social, education and vocational development. Performs patient, family and caregiver education, including motivational interviewing and patient centered health action plans. Collaborates with other resources within organization and in the community to address patient’s social determinant of health needs. Performs other duties as assigned, including supporting the CHAS Health Mission and Core Values.
