About The Position

Easterseals PORT Health (ESPH) is seeking a compassionate and dedicated full-time Mobile Crisis Coordinator to join their Community Regional Crisis Response (CR2) team in Northern Virginia. The role functions primarily to provide clinical and coordination of services for the REACH Mobile Crisis Team. This includes on-site and telephonic/telehealth crisis intervention, a shared rotation of a 24/7/365 crisis line, and crisis stabilization for individuals in their home or other community settings. ESPH's mission is rooted in empowering individuals and strengthening communities.

Requirements

Qualified Mental Health Professional (QMHP) and/or Qualified Developmental Disabilities Professional (QDDP) – Minimum of a bachelor’s degree in human services or related field from a USDOE or CHEA accredited institution and no less than one year of fulltime relevant experience.
Must be registered (or eligible to register) as a QMHP-T with the Virginia Board of Counseling or hold a valid Licensed Mental Health Professional-Resident or Supervisee.
Those not currently registered must be eligible to apply and must submit their QMHP-T application upon hire, with registration obtained within 30 days, unless they are a Licensed Mental Health Professional.
Knowledge of the MH and I/DD systems of care.
Knowledge and ability to utilize computer word processing and data programs including but not limited to Microsoft Office Suite, Web-based data entry, etc.
Must demonstrate capacity to effectively communicate verbally and in written form.
Must have a valid, unrestricted, state appropriate driver’s license and vehicle insurance.
Satisfactory background check/screening as referenced in Easter seals PORT Health Human Resources Policy A6.
Satisfactory Tuberculosis (TB) Screening
Current satisfactory completion of therapeutic interventions training - either Therapeutic Options of Virginia (TOVA) or Crisis Prevention Institute (CPI) approved curricula.
Current approved certification to provide emergency CPR/First Aid
Additional required trainings include, but may not be limited to the following: Agency Orientation, Confidentiality and HIPAA, Human Rights, Personnel and Benefits, Emergency Preparedness and Emergency Response, Person Centered Thinking, Blood Borne Pathogens and Infection Control (including Pandemic Flu), Working with Families, Overview of Dual Diagnosis (ID/MI), Overview of Autism Spectrum Disorders, Workplace Safety, Harassment, Mental Health aspects of Intellectual Disabilities, Psychopharmacology and Intellectual Disabilities, Emergency Crisis Assessments for Individuals with Intellectual Disabilities and Behavioral Health Needs, Schizophrenia and Psychotic Disorders in Persons with Intellectual Disabilities, Mood Disorders in Persons with Intellectual Disabilities, Anxiety Disorders in Persons with Intellectual Disabilities, Civil Commitment Process in Virginia (ECO/TDO), Incident Reporting, Genetic Disorders and Behavioral Phenotypes.

Nice To Haves

Graduation from an accredited college/university with a Master’s Degree in Counseling, Social Work, Psychology, or closely related field preferred.
Current, unrestricted Virginia license as an LPC, LCSW, or LMFT or closely related preferred.
Experience in intellectual/developmental disabilities, including programming for individuals with significant behavioral needs preferred.

Responsibilities

Provides rapid response to individuals experiencing a behavioral health crisis.
Meets the individual in an environment where they are comfortable to facilitate service engagement, stabilization, and resolution of the crisis when possible.
Provides appropriate care/support/supervision in order to maintain safety for the individual and others, including but not limited to crisis intervention, community stabilization, care coordination, and treatment planning.
Refers and links to all medically necessary behavioral health services and supports.
Coordinates with behavioral health providers providing services to the individual throughout the delivery of the service.
Maintains linkages and relationships with community partners.
Ensures the completion of all required documentation for Mobile Crisis services, including but not limited to assessment, treatment planning, and Crisis Education and Prevention Plans (CEPP).
Participates in required meetings with REACH Team Members and other applicable parties.
Coordinates admissions/discharges and requests for appropriate individuals to/from REACH Crisis Therapeutic Home in collaboration with the REACH CTH Supervisor.
Coordinates admissions/discharges and requests for appropriate individuals to/from REACH Mobile crisis supports in collaboration with REACH Team Lead.
Monitors individuals for any mental health or behavioral symptoms and provides crisis prevention, intervention and de-escalation supports in accordance with crisis guidelines and individual’s plan.
Plans, structures and prompts social and leisure time activities, including activities that build skills for behavior management, stress management, communication, adaptive living and daily living.
Collects and enters required data and produces documentation on administrative and service delivery and overall utilization of REACH services within established timeframes and according to set protocols.
Complies with all ESPH policies and procedures, HIPAA guidelines, DBHDS rules, Medicaid/Medicare federal guidelines.
Attends all staff and program meetings as requested.
Maintains regular attendance and punctuality, manages use of PTO time (if applicable), and follows all policies and procedures regarding time off.
