About The Position

The Mobile Clinic Coordinator (Community Health Worker) supports the delivery and expansion of Vecinos’ mobile clinic services across Western North Carolina. This position is responsible for coordinating mobile clinic operations, conducting patient outreach and navigation, and ensuring continuity of care for uninsured adults living at or below 250% of the Federal Poverty Level. The Coordinator works closely with clinical staff and community partners to reduce barriers to care and improve access to integrated primary and behavioral health services.

Requirements

  • Minimum 2 years of experience in community health, outreach, or care coordination
  • Strong organizational, communication, and interpersonal skills
  • Ability to work independently in community-based settings
  • Must have held a valid driver’s license for five years and have a clean driving record
  • Have reliable transportation
  • Be 25 years of age to legally drive the mobile clinic

Nice To Haves

  • Community Health Worker certification preferred (or willingness to obtain within 12 months)
  • Experience working with uninsured, low-income, or rural populations preferred
  • Bilingual (Spanish/English) strongly preferred

Responsibilities

  • Coordinate scheduling and logistics for mobile clinic sites across the service area
  • Prepare and maintain mobile clinic supplies, medications, and equipment
  • Support on-site clinic operations, including patient flow and documentation
  • Collaborate with providers, nurses, behavioral health staff, pharmacy, and lab teams
  • Drive the mobile clinic vehicles to and from designated mobile clinic sites and to maintenance locations
  • Conduct patient intake, eligibility screening, and enrollment
  • Schedule appointments and coordinate referrals to internal and external services
  • Facilitate follow-up care, including ensuring timely follow-up of lab results and other services
  • Maintain accurate and timely documentation in the electronic health record
  • Provide culturally and linguistically appropriate health education in both English and Spanish
  • Conduct outreach to engage uninsured individuals in need of services
  • Support patients in understanding and managing chronic conditions
  • Develop and maintain relationships with community-based organizations
  • Coordinate mobile clinic activities with partner sites
  • Support referral pathways between community partners and Vecinos services
  • Identify and address barriers such as transportation, language, and cost
  • Coordinate interpretation and transportation support as needed
  • Connect patients to social services, including food, housing, and financial assistance
