About The Position

As a Mobile Case Manager with Chartiers Center, you will have the opportunity to make a meaningful impact in the lives of adults living with serious mental illness and co-occurring conditions, including substance use disorders, and physical health challenges. Requirements

  • Associate’s degree in human services and 3+ years of mental health experience.
  • Bachelor's degree in human services and 2 years of mental health experience OR Master's degree in human services with one year of experience
  • Valid Driver’s License Required
  • Ability to travel throughout the community

Responsibilities

  • Meet with individuals in their homes and throughout the community to provide support, guidance, and service coordination.
  • Conduct comprehensive assessments to identify strengths, needs, preferences, and recovery goals.
  • Develop, implement, and monitor individualized service plans that promote stability, growth, and independence.
  • Connect individuals with critical resources, including housing, healthcare, behavioral health services, benefits, employment opportunities, and community supports.
  • Collaborate with healthcare providers, therapists, family members, and community agencies to ensure coordinated and effective care.
  • Maintain regular contact with individuals to monitor progress, address challenges, and identify emerging needs.
  • Serve as a strong advocate, helping individuals navigate systems and access the services they need to succeed.
  • Complete accurate and timely documentation in accordance with agency and regulatory requirements.
  • Participate in a shared on-call rotation, typically no more than once per month.
