About The Position

HealWell, LLC is seeking a highly organized and dependable Mobile Care Coordinator to support our growing mobile wound care team. This position serves as a central point of coordination between our providers, patients, healthcare facilities, home health agencies, DME companies, pharmacies, and internal administrative team. The Mobile Care Coordinator helps keep our field-based providers organized and ensures that schedules, orders, documentation, supplies, and clinical communications continue moving efficiently. This position is ideal for someone who is detail-oriented, comfortable managing multiple priorities, and enjoys being the person who keeps everything organized behind the scenes. The position will preferably be based in our Midwest City office, although a hybrid work arrangement may be considered for the right candidate after training and based on performance and operational needs.

Requirements

  • Previous experience in healthcare administration, medical office coordination, scheduling, referral coordination, or a similar healthcare support role strongly preferred.
  • Experience working with physicians, nurse practitioners, physician assistants, home health agencies, skilled nursing facilities, or other healthcare organizations is preferred.
  • Experience with electronic medical records and healthcare scheduling systems preferred.
  • Strong computer skills and ability to learn multiple software platforms.
  • Excellent written and verbal communication skills.
  • Exceptional attention to detail and follow-through.
  • Ability to prioritize multiple competing responsibilities in a fast-paced environment.
  • Strong problem-solving skills and the ability to work independently.
  • Professional, dependable, and comfortable communicating with healthcare providers and outside organizations.
  • Candidates must have a valid driver's license, current automobile insurance, and reliable transportation.

Nice To Haves

  • Wound care experience is helpful but not required.
  • Bachelor's degree preferred but not required for candidates with relevant healthcare experience.
  • This role requires someone who notices when something hasn't been completed and takes the initiative to follow through.
  • The ideal candidate is organized, proactive, dependable, and able to keep track of multiple moving pieces without losing attention to detail.
  • You should be comfortable communicating with healthcare providers, managing frequent changes throughout the day, and following a task from the initial request through completion.

Responsibilities

  • Manage and coordinate daily and future schedules for mobile healthcare providers.
  • Confirm patient appointments and coordinate new patient evaluations and follow-up visits.
  • Communicate schedule changes, patient updates, and other time-sensitive information to providers.
  • Serve as a primary administrative support resource for assigned providers.
  • Coordinate communication between providers, patients, healthcare facilities, home health agencies, pharmacies, DME companies, and other members of the care team.
  • Process provider orders, including wound care orders, DME, wound vac supplies, medications, vascular studies, and other diagnostic or treatment-related requests.
  • Coordinate wound measurements, documentation, and supporting information required by vendors and outside agencies.
  • Assist with prescription coordination and other provider-directed clinical support activities within the scope of the position.
  • Track outstanding requests and follow through to ensure completion.
  • Assist providers with clinical documentation workflows within the electronic medical record.
  • Track outstanding documentation and follow up regarding incomplete progress notes or required information.
  • Upload clinical documents and wound photographs into appropriate systems.
  • Obtain provider signatures and distribute completed orders and documentation.
  • Send progress notes, wound care orders, and other records to healthcare agencies and facilities.
  • Maintain accurate records of transmitted documents and follow up when necessary.
  • Process release-of-information requests and maintain appropriate documentation.
  • Monitor shared inboxes, phone systems, messages, and provider requests.
  • Respond to and route incoming communications appropriately.
  • Assist with insurance and patient information verification as needed.
  • Maintain accurate and current patient care coordination information.
  • Identify delays or barriers to care and communicate concerns to the appropriate team member.
  • Participate in team meetings and workflow/process improvement initiatives.
