We are seeking a dedicated and compassionate Mobile Mental Health Licensed Professional to join our team. This role involves providing mental health services to individuals in various community settings, including homes, schools, and other locations. The ideal candidate will have a strong understanding of mental health issues and be able to deliver care with empathy and professionalism.

Requirements

Strong verbal and written communication skills
Flexibility with hours as may require some evenings and weekends
Master’s Degree AND NYS Licensure LCSW or LMHC
1 to 3 years’ experience preferably with individuals with mental health diagnosis
Must be capable to sit or stand in front of a computer for long-periods of time
Able to work in open space floor plan
Must be capable to move throughout work day and or meet individuals where they are at in the community)
Work alongside co-workers within 3 feet
Must be able to move in tight spaces
Occasional lifting of > _25+ pounds

Nice To Haves

Bilingual English and Spanish speaking a plus

Responsibilities

Clinician will respond to mental health crises and provide immediate assessment, support and intervention.
Clinicians are required to be skilled in the clinical triage and assessment of all emergency, urgent, and routine calls
Knowledgeable about County resources with particular knowledge of the services of the Agency
Be able to assess needs of each individual served and intervene as appropriate to the situation
Be able to coordinate outreach services, service planning, follow-up support and administrative back up as needed and clinically appropriate
Work in compliance with MMHT, agency and regulatory procedural guidelines
Conduct assessments and evaluations of clients' mental health needs in the community.
Collaborate with other healthcare providers, social services, Law enforcements and community resources to coordinate care.
Maintain accurate and up-to-date client records and documentation.
Educate clients and their families about mental health conditions and coping strategies.
Advocate for clients' needs and rights within the community and healthcare system.
Coordinate follow up response to ensure stabilization.
Perform other duties as assigned
