The facility's Point of Contact as the Military Care Coordinator for Active Duty Service Members and Veterans in any of the facility's programs. Establishes and monitors admission and discharge protocol to ensure quality and consistency. Attends acute, New Choices, and outpatient staff meetings to keep abreast of programs, changes, and gives input of quality of care, transportation, admissions, and discharges. Attends and participates in acute, New Choices, and outpatient multi-disciplinary treatment team meetings/rounds daily where Active Duty Service Members and Veterans are receiving treatment to ensure appropriate treatment planning and protocol are conducted. Ensures compliance with individual military bases referral, communication and discharge planning expectations in partnership with Cedar Springs Case Management. Works closely with medical staff acting as a liaison between, MD, patient, UM case management and respective base command structures. Monitors (quality assurance checks) program integrity to ensure that all services are documented as per the programs daily schedule and documented in a timely fashion. Discrepancies will be reported to the Program Director. Provide feedback related to overall program effectiveness to Clinical/Program Director and Cedar Springs Senior Management as needed. Performs public speaking, outreach and development functions in support of marketing department, to maintain positive and open relations with all military contacts/bases. Coordinates with Cedar Springs Marketing Department to identify, expand and develop opportunities to grow programs.
