About The Position

Provide Mobile Integrated Health (MIH) services to patients in non-traditional settings, including homes, community sites, and transitional care environments. This role involves performing patient assessments, developing and implementing care plans, providing health education, performing point-of-care testing, initiating treatments, and offering follow-up care for recently discharged patients. The MIH Clinician will also identify social determinants of health, coordinate referrals to community resources, communicate patient status to healthcare providers, and document care in the electronic health record. Participation in care coordination activities and data collection for program outcomes is also a key responsibility. Additionally, assisting with the orientation and mentoring of new MIH staff and maintaining MIH equipment and supplies are part of the role.

Requirements

  • Current Paramedic certification or Registered Nurse license in the State of Iowa.
  • Minimum of three (3) years of pre-hospital experience.
  • Valid driver’s license with acceptable driving record.
  • Ability to perform medication administration within scope of practice.
  • Ability to complete clinical documentation using electronic health records.
  • Basic computer skills required.
  • Good verbal and written communication skills are essential.
  • Annual Infection Prevention education requirements as designated in IC#23 Infection Prevention Education Requirements

  • Community Paramedic certification preferred.

Responsibilities

  • Provide Mobile Integrated Health services to patients in non‑traditional settings including homes, community sites, and transitional care environments.
  • Perform patient assessments to identify medical needs, functional limitations, and psychosocial factors affecting health.
  • Develop and implement individualized plans of care in collaboration with physicians, nurses, and other members of the care team.
  • Provide health education related to disease management, medications, and treatment plans as appropriate.
  • Perform point‑of‑care testing and clinical interventions within scope of practice and approved protocols.
  • Initiate treatments and interventions as permitted by standing orders and medical direction.
  • Provide follow‑up care for patients recently discharged from the emergency department or hospital.
  • Identify social determinants of health and coordinate referrals to appropriate community resources.
  • Communicate patient status, progress, and concerns to healthcare providers in a timely manner.
  • Document clinical care and patient responses accurately and timely in the electronic health record.
  • Participate in care coordination activities with hospital departments, primary care providers, EMS, behavioral health, and social services.
  • Collect information and data to support program outcomes and quality improvement initiatives.
  • Assist with orientation and mentoring of new MIH staff as assigned.
  • Maintain MIH equipment and supplies in a clean and functional state.
