About The Position

As a Midlevel Provider at CARES 360 Memphis, you will play an important role in expanding access to HIV treatment, prevention, STI services, and sexual healthcare through the CARES 360 Memphis low-barrier clinic. You will serve as an accessible entry point to care for individuals who may be newly diagnosed with HIV, looking to begin PrEP, seeking STI treatment, or reconnecting with HIV care. You will provide timely clinical services, answer patient questions, initiate appropriate treatment, and help connect patients with ongoing care and supportive resources. This role incorporates walk-in access to help reduce delays and barriers to care, working within established clinical protocols while collaborating with case management, prevention, behavioral health, pharmacy, phlebotomy, and other supportive services. We are looking for a provider who is compassionate, clinically confident, adaptable, and collaborative, with an interest in taking initiative and contributing ideas as the clinic grows.

Requirements

  • Current Nurse Practitioner or Physician Assistant licensure in Tennessee, or ability to obtain appropriate Tennessee licensure prior to hire
  • DEA registration and appropriate prescriptive authority required prior to hire
  • Strong clinical judgment and ability to work independently within scope of practice
  • Excellent interpersonal, communication, and patient-engagement skills
  • Ability to provide compassionate, culturally responsive, trauma-informed, and affirming care
  • Ability to adapt to patient needs within a walk-in clinical environment
  • Strong collaboration and teamwork skills
  • Ability to maintain accurate and timely clinical documentation
  • Ability to take initiative while working effectively within established clinical protocols

Nice To Haves

  • Previous experience providing HIV treatment and/or prevention services
  • Experience with PrEP, PEP, ART, STI treatment, or sexual healthcare
  • AAHIVM credentialing
  • Experience in public health, sexual health, women's health, infectious disease, community health, or another related clinical setting
  • Experience working with underserved or historically marginalized populations
  • Experience working as part of a multidisciplinary healthcare team
  • Interest in contributing to the development and growth of an expanding clinical program
  • Candidates who do not have extensive HIV-specific experience but bring relevant clinical experience and a strong interest in developing expertise in HIV treatment and prevention are encouraged to apply.

Responsibilities

  • Initiate antiretroviral therapy (ART) for individuals newly diagnosed with HIV
  • Re-engage individuals who have fallen out of HIV care and help address barriers to treatment
  • Provide bridge medications and appropriate follow-up until patients are successfully connected to ongoing care
  • Initiate pre-exposure prophylaxis (PrEP) for eligible patients
  • Provide post-exposure prophylaxis (PEP) services
  • Educate patients on HIV treatment, prevention, medication adherence, and available resources
  • Provide STI screening, diagnosis, and treatment
  • Provide DoxyPEP when clinically appropriate
  • Order and review laboratory testing and communicate results to patients
  • Provide appropriate short-term follow-up based on individual patient needs
  • Use established clinical protocols and sound clinical judgment to provide evidence-based care
  • Maintain accurate and timely documentation within the electronic medical record
  • Serve as an accessible clinical entry point for individuals seeking HIV treatment, prevention, and sexual health services
  • Help patients navigate new diagnoses and understand available treatment and prevention options
  • Coordinate referrals and transitions to ongoing care with the CARES 360 Memphis clinical team
  • Work closely with case managers and prevention staff to reduce barriers and help patients successfully connect with needed services
  • Provide culturally responsive, trauma-informed, and affirming care
  • Collaborate with case management, prevention staff, behavioral health, pharmacy, phlebotomy, front-desk staff, and other community partners
  • Work closely with the Medical Director and clinical leadership to provide coordinated patient care
  • Participate in quality improvement efforts and the continued development of the low-barrier clinic model
  • Identify opportunities to improve workflows, referral processes, rapid-treatment pathways, and the overall patient experience
  • Take initiative in recommending solutions and improvements as services continue to grow
