Requirements

  • Bachelor’s level required in Social Work or relevant degree, Psychology, Sociology, including Education degree due to the nature of mentoring
  • Must have 2 years of relevant experience in child welfare, social services, or a related field and a desire to work with children and families.
  • Excellent writing, communication, interpersonal, problem-solving, time management and organizational skills.
  • Ability to work independently and as part of a team.
  • Proven self-management abilities including meeting deadlines, prioritizing multiple tasks efficiently, timely completion of documentation and maintaining accurate and up-to-date case records.
  • Ability to work effectively with a diverse, multidisciplinary team and connect with a diverse client population to engage them in services throughout treatment.
  • Ability to receive and execute feedback to professionally grow and/or improve and be flexible with programmatic needs and changes.
  • Ability to manage stress, self-care and maintain a high level of self-awareness.
  • An understanding and commitment to complying with the New York State Central Register of Abuse and Neglect protocol (requirement to report any suspected abuse, maltreatment or neglect of any child).
  • Must be comfortable and able to travel and visit families in their homes and perform safety assessments.
  • Bachelor’s Degree from a U.S. accredited educational institution or Credential Evaluation for a non-U.S. educational institution

Responsibilities

  • Under the supervision of the AMP Supervisor, Mentors work with youth to instill core values such as problem solving, communication, responsibilities, and time management to address problematic behavior and encourage change.
  • Provide home based services to youths via individual or group sessions to decrease risk and safety factors and meet their goals and needs.
  • Engage primary caregivers and other key participants throughout treatment to engage in sessions and overcome barriers to engagement when they arise to ensure successful goal attainment.
  • Complete timely and accurate documentation of session progress and collateral notes in all databases that includes Evolv, Connections, Promis and/or Care4 to support treatment fidelity, tracking, and planning.
  • Establish and maintain collaborative working relationships with colleagues, community providers and model/external stakeholders to connect families to resources and support ongoing treatment communication and collaboration for effective treatment planning.
  • Continuously assess for families safety, addressing crises, completing safety plans and incident reports in a timely manner as necessary to ensure the safety and well-being of all families served by the program with an emphasis on decreasing system involvement (could include after hour/on call support).
  • Conduct intake thorough the completion of initial assessments and gather essential consent forms for new cases, ensuring timely case openings, completeness of information and family understanding of services to achieve a smooth start to treatment.
  • Participate in group, individual supervisions, trainings, all staff meetings and community outreach events to strengthen professional development, skill growth, group cohesion and community visibility.
  • Organizational expectations include: Regular and punctual time and attendance, and adherence to The Foundling's and your department’s specific time and attendance policies and protocol.
