About The Position

Merakey is seeking a Mental Health Worker to join their team in Monaca, PA, supporting Beaver County. This role involves providing rehabilitation, support, and direct care services to individuals with mental health and behavioral health needs. The position requires implementing treatment plans under clinical supervision, collaborating with various stakeholders, ensuring client safety, and participating in interdisciplinary treatment planning. The role is part of an Assertive Community Treatment (ACT) team that provides comprehensive services to adult individuals with serious mental illness (SMI), including therapy, medication management, and resource linkage. Services are delivered in the individual's home or community, with at least three home visits per week. The position requires the use of personal vehicles for client transportation and community-based work.

Requirements

• Bachelor's Degree in a human service field
• 1 year experience working with individuals in a recovery setting and/or crisis experience.
• Valid PA Driver's License. Travel is necessary to offer support across Beaver County.

Responsibilities

• Provide rehabilitation, support, and direct care services to individuals with mental health and behavioral health needs, including assistance with daily living activities and therapeutic interventions.
• Implement treatment plans under clinical supervision, including substance use services, education, family consultation, and crisis intervention support as needed.
• Collaborate with families, healthcare providers, social service agencies, and community partners to coordinate comprehensive care and support positive outcomes.
• Ensure the safety and well-being of individuals served by following established policies, procedures, and safety protocols.
• Participate in treatment planning and work collaboratively with the interdisciplinary team to support recovery and treatment goals.
