About The Position

The Mental Health Support Worker in the Psychiatric Rehabilitation Department manages an assigned client caseload and designs, develops, and leads skill-training groups, adapting curriculum as client needs change. The role includes identifying client needs, strengths, and goals and helping develop plans to address them, while maintaining timely and accurate client records in accordance with state charting requirements. The worker coordinates with treatment staff and other agencies, supports client referrals and transportation access, and participates in staff meetings, development, and ongoing training. Initial and ongoing training are provided by a supervisor or assigned team member. Employees are expected to become proficient in the position within six months.

Requirements

High school diploma or equivalent.
Two years of experience in the mental health field.
Comfort with crisis intervention.
Some knowledge of major diagnostic categories of mental illness.
Strong interpersonal and communication skills.
Ability to work effectively as a team member.

Responsibilities

Manage an assigned caseload and present cases to the team.
Identify client needs, problems, strengths, and goals, and participate in developing plans to meet those goals.
Compile and maintain timely, accurate case records, including daily progress notes, service plan reviews, referral forms, chart audits, intake forms, and other necessary forms.
Lead group sessions and assigned skill-training activities.
Communicate with NTC treatment staff and staff from other agencies about mutual clients when appropriate and necessary.
Attend and participate in inter-agency meetings regarding mutual clients as needed.
Assist assigned clients with referrals to needed community services.
Participate in staff development activities to enhance skills and education.
Use NTC’s Electronic Health Records (E.H.R.) during employment.
Help assigned clients access transportation through the Endless Mountains Transportation Authority.
Complete 18 hours of training per calendar year, including 12 hours specific to Psychiatric Rehabilitation.
Complete six hours of shadowing on the Psychiatric Rehabilitation floor.
Transport clients to and from the program when requested if transporters are unavailable or scheduled off.
Complete weekend day trips for consumers when assigned at the manager’s discretion.
Attend weekly staff meetings and general staff meetings as scheduled.
Participate in ongoing in-service training programs.
Become proficient in the position within six months and request additional training or education through a supervisor, manager, or director when needed.
Perform other assigned functions and duties.
