About The Position

Phoenix Rising Recovery is currently hiring a Case Manager for our Mental Health Outpatient Treatment Program! The Case Manager provides direct support and care coordination for clients receiving outpatient treatment for mental health and substance use disorder diagnoses. The Case Manager assists clients with treatment engagement, discharge planning, connecting to community resources, life skills and goal development, while supporting a safe, structured, and therapeutic treatment environment. This role works collaboratively with a multidisciplinary treatment team to help clients build stability, increase independence, and successfully transition to lower levels of care and community living.

Requirements

  • One year full-time Case Management experience working directly with mental health clients, preferably in a treatment setting
  • Knowledge of and belief in the principles of client centered treatment.
  • Demonstrated ability to work independently and as a team player.
  • Excellent communication skills both oral and written.
  • Demonstrate professionalism and maintain strong boundaries with all clients and staff.
  • Must be fluent in speaking, reading, and writing English.
  • Valid driver's license.
  • Current First Aid and CPR certification or willing to obtain upon hire.

  • Masters Degree preferred
  • Substance Abuse Counseling Certificate preferred

Responsibilities

  • Support clients with scheduling appointments, transportation coordination, housing and employment resources, and other psychosocial needs
  • Coordinate care and maintain regular communication with clients, families, referral sources, outside providers, and community agencies
  • Assist clients with developing and progressing toward individualized treatment goals
  • Support clients in developing independent living skills, healthy coping strategies, and community reintegration skills
  • Facilitate discharge planning, aftercare coordination, and linkage to community resources
  • Maintain accurate, timely, and compliant clinical documentation within the electronic medical record
  • Participate in multidisciplinary treatment team meetings and provide client updates
  • Provide psychoeducation, support, advocacy, and encouragement to clients
  • Assist in crisis intervention and de-escalation when necessary
  • Facilitate groups as needed
  • Collaborate with the treatment team including therapists, medical providers, nursing and direct care staff
  • Participate in ongoing trainings, supervision, and quality improvement initiatives
