About The Position

Valor Healthcare is seeking a Licensed Clinical Social Worker (LCSW) or equivalent counselor/therapist (LPC or LMFT) to join the mental health team at the Veteran Affairs (VA) Community Based Outpatient Clinic (CBOC) in Columbia, TN. This role involves providing therapy and counseling to an all-adult veteran patient population. Requirements

  • Must have a Doctoral degree in psychology from an APA, PCSAS, or CPA accredited graduate program, OR a Master's degree or higher in Social Work from a Department of Education registered program with specific training dealing with adult patients. Equivalent degrees (Counseling, Marriage and Family Therapy) will be considered and are subject to final VA approval.
  • Must comply with and maintain all requirements for a valid, unrestricted LCSW (or equivalent) license in the state of desired employment and be able to independently practice at the master’s degree level.
  • Minimum of 1-3 years' post-master direct counseling experience with adult patients.
  • Must be credentialed and remain in good standing through the Veterans Health Administration (VA).
  • Current certification in Basic Life Support (e.g., AHA valid for two years).
  • Strong computer skills, including EMR experience.

Responsibilities

  • Performs initial and follow-up evaluations, including assessments.
  • Engages the veteran, core, and expanded PACT team members in treatment planning.
  • Provides a range of treatment services, including individual therapy, typically focused on behavior activation and short-term needs for physical and behavioral health issues (e.g., Diabetes, Heart Conditions, Depression, Anxiety, Adjustment Disorders).
  • Provides evidence-based psychotherapies as necessary and appropriate.
  • Provides emergency care, including referring patients for emergency primary care, psychiatric medication evaluation, or coordinating inpatient psychiatric unit admissions.
  • Provides crisis intervention services, addressing causes and presenting complaints, coordinating family conferences, and serving as a liaison to family members.
  • Provides case management to Veterans and families as needed throughout the continuum of care.
  • Addresses issues such as advance directives and organ donation in accordance with acceptable standards.
  • Inputs all patient information into VISTA/CPRS.
  • Completes any and all clinical reminders “due” at the time of each patient visit.
  • Participates in program planning and development, making recommendations for changes in policies and procedures.
  • Collaborates in performance improvement processes and complies with performance measures required by Valor and the VA.
  • Participates in activities that obtain feedback from veterans and works to enhance services.
  • Complies with all Valor and VA training requirements.
  • Fulfills compliance requirements of the Office of Inspector General (OIG), Joint Commission (JC), Environment of Care (EOC) oversight, lab compliance, and other related items.
  • Remains focused on achieving excellent clinical outcomes through the specified VA guidelines.
  • Provides excellent customer service to each veteran and his/her family, colleagues, and clinic visitors.
  • Collaborates with Clinic leadership to resolve veteran concerns or complaints.
  • Participates in the clinic’s outreach events to support enrollment initiatives.
  • Supports patient enrollment and retention in appointments, follow-up visits, or nurse visits.
  • Embraces and supports new initiatives, whether clinical or operational.
  • Other duties as assigned.
