About The Position

The FACT Mental Health Professional functions as a member of a multidisciplinary team, with primary responsibility for providing clinical assessment, therapeutic interventions, and crisis response, including next-day follow-up, alongside coordinated treatment and rehabilitation services. This role supports client recovery through skill development, symptom management, and goal-focused care. The FACT team at KMHS provides time-limited services to clients most impacted by the symptoms of their mental health disorders, such as schizophrenia and schizoaffective disorders. The FACT team includes a case manager, a therapist, a peer specialist, a vocational specialist, and a nurse, as well as a clinical supervisor and a psychiatric provider for medication management. Client Caseloads are small to assure individualized attention to client needs, until their treatment goals are reached, and are then transferred back to Adult Outpatient services. Most services are provided to clients in their homes or other community settings.

Requirements

  • Master's degree in psychology, social services, or behavioral health field.
  • Completion of supervised clinical field practicum during graduate program or two (2) years’ experience working in behavioral health field prior to graduate degree.
  • Agency Affiliated Counselor Registration.
  • Must meet and maintain the definition of a Mental Health Professional as defined in RCW 71.05.020.
  • Must meet all agency driving requirements, including being at least 21 years old, having been licensed for a minimum of three years, successfully completing a motor vehicle history check, possessing and maintaining a current, valid driver’s license in the state of Washington, and having reliable, insured transportation.

  • Ability to obtain or currently possess a Washington State associate or independent clinical license (LICSW-A, LMHC-A, LMFT-A, LICSW, LMHC, LMFT).
  • Certified Psychiatric Rehabilitation Practitioner’s credential.
  • Kitsap Transit Driving Permissions for Kitsap Transit van (requires annual background check through Kitsap Transit and on-site, hands-on driving training.

Responsibilities

  • Provide clinical assessment, therapeutic interventions, and rehabilitation services in accordance with psychiatric rehabilitation and recovery-oriented principles.
  • Proactively respond to non-emergent crises, as supported and assigned by supervisor, to offer greater assessment, intervention, and coordination of care with timely next-day follow-up as indicated by client need.
  • Develop, implement, and update risk assessments as well as safety plans following crisis intervention with coping skills and symptom management strategies aligned with client's established treatment plan and current life skills.
  • Participate in individualized treatment planning for assigned clients.
  • Use recovery-oriented language and clinical strategies to build client’s ability to function independently in the community and enjoy their desired quality of life. Assist clients in building skills to independently access the resources they need over a period.
  • Engage and motivate clients to make behavioral changes that support long-term recovery.
  • Provide psychoeducation to clients and their families or natural supports, where needed, about their mental health diagnosis and the recovery process for those with psychotic disorders.
  • Document client progress in timely way. Documentation should demonstrate how therapeutic activities are relevant to client’s treatment plan goals.
  • Attend daily FACT Team meeting to provide a systematic review of each client's progress (up to 25 assigned), while promoting collaborative communication and the sharing of critical updates or ideas that support person-centered, recovery-oriented care.
  • Collaborate with individual clients, the FACT Team Lead, and/or FACT Operations Manager to identify and facilitate referrals to additional specialized KMHS services or community partners offering services aligned with person-centered, recovery-oriented care.
  • Support clients in reducing or abstaining from the use of substances and help teach clients sober coping skills. FACT uses a harm reduction approach and links clients with KMHS co-occurring treatment as needed.
  • Use situational awareness to evaluate and reduce physical risk factors for self, team members, and clients when in potentially unsafe homes or community settings, or when around potentially unsafe individuals.
  • Coordinate with other team members to participate in crisis intervention, as directed by supervisor. Communicate with Mobile Crisis Outreach Team and law enforcement officials when appropriate.
  • Assist clients in building skills for independent living in the community including, but not limited to, shopping, cooking, cleaning, personal hygiene, budgeting, socialization, and accessing transportation.
  • Serve as a primary clinician for up to 25 clients, completing all treatment planning and all documentation needs.
