About The Position

Merakey is seeking a Mental Health Professional to join their team in New Orleans, LA. This role is an essential part of the Assertive Community Treatment (ACT) Team, a multidisciplinary team providing evidence-based services to individuals diagnosed with severe and persistent mental illness in the greater Baton Rouge area. The Mental Health Professional will provide individual and group therapy, create person-centered treatment plans, review and update individual crisis plans, and complete psychosocial assessments. This role also requires maintaining thorough and timely clinical documentation, including progress notes, assessments, and interpretative summaries, as well as providing crisis intervention and support.

Requirements

• Must have an active driver's license within the state of Louisiana
• Willing to work in a community based setting
• Must have an active PLPC/LMSW within the state of Louisiana.

Responsibilities

• Provide individual therapy and group therapy to consumers in service.
• Create the person-centered treatment plan.
• Review and change the individual crisis plan as required.
• Complete psychosocial assessments.
• Maintain thorough and timely clinical documentation such as: progress notes, assessments, and interpretative summaries.
• Provide crisis intervention and support.
