About The Position

This position is located at the SC Department of Behavioral Health and Developmental Disabilities (DBHDD) - Office of Mental Health, Spartanburg Area Mental Health Center, 250 Dewey Ave, Spartanburg, SC 29303. Under limited supervision, serves as the Center's Housing Coordinator to ensure patient access to appropriate community-based housing. Complete assessment of patient living Community Care Residential Facilities (CRCF's). Assess patients' psychiatric status to determine the ability to move to a lower level of care in the Housing continuum.

Requirements

  • A master's degree and professional experience in guidance, counseling or the social sciences and two (2) years of post-graduate experience in the clinical counseling program.
  • Must be licensed or eligible for licensure in SC and willing to work towards licensure in SC within the first six months of employment.
  • Employee must be able to meet BHDD-OMH credentialing requirements.
  • Applicants must provide evidence of clinical experience providing direct patient care.
  • Successfully complete the Basic Fundamentals course with a minimum grade of 75.
  • Be able to pass CPR and Managing Agitation in the Psychiatric Setting (MAPS) Certification.
  • Knowledge of psychopathology and client management within the community-based psychiatric setting.
  • Experience working with Chronically mentally ill adults and seriously emotionally disturbed children and youth.
  • Organizational and communication skills are required for this position.

Responsibilities

  • Provide oversight for the Center's housing programs, including Community Housing, Homeshare Respite, TLC/Housing and Homeless Supportive Housing developments, Community Residential Care Facilities (CRCF's) and other supportive housing programs, as applicable.
  • Coordinate administrative requirements of the agency's housing programs, including payment contracts and patient agreements, outreach and recruitment efforts for Homeshare providers, and completion of assessments for patients living in CRCF's.
  • Assesses patients' psychiatric status to determine ability to a lower level of care in the Housing continuum.
  • Coordinates with the agency's Transition Programs staff, including Clinical Care Coordinates (CCCs) and Transition Specialists, MCO, CCCs, Certified Peer Support Specialists, and Benefits/Entitlement Specialists.
  • Partners with the local HUD Continuum of Care, public housing authority(ies), and affordable housing owners and property managers.
  • Coordinates patient housing referrals and move-ins and maintains wait lists for each Center housing program.
  • Enter and maintain Community Housing data in the agency's Electronic Health Record.
  • Prepare and submits the monthly Community Housing report.
  • Enter and maintain Center Homeshare program and patient information in the agency's SharePoint sited and CIS/Electronic Health Record.
  • Participate in other OMH or partner agency programs and activities as may be required.
