This position is located at the SC Department of Behavioral Health and Developmental Disabilities (DBHDD) - Office of Mental Health, Columbia Area Mental Health Center (CAMHC), Emergency Services - Housing Coordinator, CAMHC #497. As the Mental Health Professional, you will serve as the CAMHC Housing Coordinator to ensure patient access to appropriate community-based housing. Services are delivered in the community and patients' homes.

Requirements

• A master's degree in guidance, counseling, the social sciences or a related field.
• Must be licensed or eligible for licensure in SC and willing to work towards licensure in SC within first six months of employment.
• In order to be credentialed for core services as a Mental Health Professional at the SC Office of Mental Health, applicants must provide evidence of clinical experience providing direct patient care . This experience may be documented as a practicum or internship during graduate program, or post-graduate clinical experience.

• Bilingual abilities in English and Spanish (or another language) are a plus.
• Have practical therapeutic knowledge of principles, methods and counseling practices and procedures for the diagnosis and treatment of behavioral and mental disorders.
• Have the ability to listen and understand and communicate information and ideas so that others will understand.
• Have knowledge of psychopathology, family dynamics, family and group psychotherapy.
• Have the ability to identify and effectively utilize resources available in the community to accomplish objectives.

• Have the ability to present clear and comprehensive oral and written reports.
• Be able to lift 5 pounds and operate a state vehicle.
• Be able to pass CPR and Managing Agitation in the Psychiatric Setting (MAPS) Certification.

Responsibilities

• Provide oversight for CAMHC housing programs, including Community Housing, TLC/Housing and Homeless Program Supportive Housing developments, Community Residential Care Facilities (CRCF's), and other supportive housing programs, as applicable.
• Coordinate administrative requirements of the agency's housing programs, including payment contracts and patient agreements.
• Coordinate with the agency's Transition Programs staff, including Clinical Care Coordinators (CCCs) and Transition Specialists, MCO CCCs, Certified Peer Support Specialists, and Benefits/Entitlement Specialists.
• Partners with the local HUD Continuum of Care, public housing authority (i.e.), and affordable housing owners and property managers.
• Coordinate patient housing referrals and move-ins and maintain wait lists for each Center housing program.
• Participate in other OMH or partner agency programs and activities as may be required.
• Attend treatment team meetings as needed and provide consultation regarding complex cases as appropriate.
• Complete documentation according to QA/OMH standards, complies with Corporate Compliance policy and Risk Management Plan, and ensure timely completion of prior authorizations.
• Enter and maintain Community Housing data in the agency's Electronic Health Record.
• Prepare and submit the monthly Community Housing report.
• Enter and maintain Center Homeshare/TLC program and patient information in the agency's SharePoint site and CIS/Electronic Health Record.
• Comply with the Office of Mental Health Compliance Plan and all legal responsibilities and requirements.
