About The Position

This position is located at the SC Department of Behavioral Health and Developmental Disabilities (DBHDD) - Office of Mental Health, Lexington County Community Mental Health Center, 301 Palmetto Park Blvd, Lexington, SC 29072. The Office of Mental Health is dedicated to providing compassionate and accessible care to the community and is seeking a motivated Mental Health Professional/Housing Coordinator to help individuals and families navigate life's challenges and achieve long-term wellness.

Requirements

  • A bachelor's degree in social work from a program accredited by a nationally recognized accrediting body for social work programs, or from a social work program whose standards are at least equivalent to the minimum standards required by the nationally recognized accrediting body approved by the Board.
  • Ability to lift 5lbs and operate a state vehicle.
  • Requires a flexible work schedule.
  • Services are delivered in the community and patient's home.
  • In order to be credentialed for core services as a Mental Health Professional at the SC Office of Mental Health, applicants must provide evidence of clinical experience providing direct patient care. This experience may be documented as a practicum or internship during graduate program, or post-graduate clinical experience.

  • Bilingual abilities in English and Spanish (or another language) are a plus.
  • Know of psychopathology and client management within the community-based psychiatric setting

Responsibilities

  • Provide oversight for the Center's housing programs, including Community Housing, Homeshare, Homeshare Respite, TLC/Housing and Homeless Program Supportive Housing developments, Community Residential Care Facilities (CRCF's), and other supportive housing programs, as applicable.
  • Coordinate administrative requirements of the agency's housing programs, including payment contracts and patient agreements, outreach and recruitment efforts for Homeshare providers, and complete assessments for patients living in CRCF'S.
  • Assess patient psychiatric status to determine ability to move to a lower level of care in the Housing continuum.
  • Coordinate with the agency's Transition Programs staff, including Clinical Care Coordinators (CCCs) and Transition Specialists, MCO CCCs, Certified Peer Support Specialists, and Benefits/Entitlement Specialists.
  • Partner with the local HUD Continuum of Care, public housing authority(ies), and affordable housing owners and property managers.
  • Coordinate patient housing referrals and move-ins and maintain wait lists for each Center housing program.
  • Enter and maintain Community Housing data in the agency's Electronic Health Record.
  • Prepare and submit the monthly Community Housing report.
  • Enter and maintain Center Homeshare program and patient information in the agency's SharePoint and CIS/Electronic Health Record.
