About The Position

The Mental Health Practitioner will join the Housing Navigation Program, providing services to individuals experiencing long-term homelessness who have mental illness and/or substance use disorders. The program assists clients in finding independent housing and offers supportive services to help them maintain it. The practitioner will lead the housing search, provide support, and advocate to overcome barriers. This role involves outreach, housing transition, and tenancy sustaining services for vulnerable adults, with a primary focus on helping individuals restore skills essential for managing their mental illness and treatment to support independent living.

Requirements

  • Bachelor's degree in behavioral health sciences plus 2,000 hours of work experience providing supportive services to vulnerable populations (people experiencing homelessness, unstable housing and/or people with co-occurring substance use disorders and mental illnesses).
  • OR No college degree and 4,000 hours of supervised work experience providing supportive services to vulnerable populations (people experiencing homelessness, unstable housing and/or people with co-occurring substance use disorders and mental illnesses).
  • Must have a car, auto insurance, and a driver's license.

Responsibilities

  • Provide instruction and monitoring on identified service areas from the client’s Housing Focused Person Centered Plan, other assessments, and recommendations from the client’s treatment team.
  • Engage in completing Housing Focused Person Centered Plans with referred individuals.
  • Continuously monitor progress and modify the Housing Support Plan and Housing Support Crisis Plan as appropriate.
  • Communicate changes in service needs and recommendations to the Clinical Supervisor/Mental Health Professional, Case Manager, and the rest of the treatment team on an ongoing basis to coordinate care.
  • Maintain records of progress for each face-to-face meeting and telephone contact with the client, including an assessment of the client’s ability to function and manage in their own apartment.
  • Assist clients in creating a plan to work towards service goals in regular meetings.
  • Schedule appointments to assist clients with requested services for the agreed-upon time and frequency in their support plan.
  • Bill for services by completing contemporaneous documentation as required by program policies and procedures.
  • Intervene as necessary for emergencies or when a person is assessed to be in crisis.
  • Connect individuals to appropriate emergency services, doctors, staff, and emergency family contacts when necessary.
  • Coordinate and cooperate with other agency and community providers, including landlords and other community resources, to provide the best possible care for the individual.
  • Complete all necessary Release of Information and documents for communication with other providers and keep them current for each person served.
  • Maintain records of contact with other providers regarding care needs.
  • Assist individuals served in obtaining and maintaining benefits, including housing subsidies and other economic assistance.
  • Provide assistance in budgeting monthly funds to maintain housing.
  • Transport clients in the community.
