The Mental Health Outreach Worker serves as the backbone for the service coordination and will assist clients transitioning from prison or jail back into the community by providing support, guidance, and service coordination. Mental Health Outreach Workers provide the direct outreach to clients ensuring both the overall health of the client and basic needs are met within the program. This includes providing clients with transportation to mandated court cases, speaking on clients’ progress to the court, facilitating group rehabilitation and weekly check-ins, barrier removal, community reintegration via public outings, and discharge planning to ensure successful placement into lower-level care facilities.
