The Brockton Multi Service Center Adult Community Clinical Services (ACCS) Integrated Team (IT) Program operated by the Department of Mental Health (DMH) is seeking a dynamic candidate who will serve as a Community Outreach Worker (MHW II) within one of our Adult Community Clinical Services (ACCS) teams. The ACCS MHW II will provide trauma-informed, person-centered community-based interventions that target skill development to assigned caseload of community clients. Will serve as the direct care liaison to assigned caseload and will provide services to assigned clients on an ongoing and as needed basis. Participates in the development of Community Service Plan (CSP) with assigned caseload. Completes required documentation and communicates critical & pertinent information to team. These positions are 40 hours per week. The selected candidate will work a rotating schedule: Weeks 1 & 2: Monday through Friday 8:30am - 5:00pm (Saturday and Sunday off). Week 3: Tuesday through Saturday 8:30am - 5:00pm (Sunday and Monday off).
