The Mental Health Housing Clinician is part of HOPE (Housing Outreach Preventing Eviction), a team that supports clients in Vancouver to maintain their housing. HOPE focuses on offering a short-term intervention based on intensive deep-cleaning and/or household management supports that address immediate risks of eviction, life safety issues, or barriers to discharge from hospital. The Mental Health Housing Clinician plays a key role in overseeing assessment, intervention, and discharge planning for all HOPE clients. Within the context of a client and family centered care model and in accordance with the standards of practice, and the established vision and values of the organization, works as part of an interdisciplinary team to place clients with mental illness in appropriate housing. Performs initial screening and conducts mental health housing assessments to determine the appropriate housing model based on clients needs, housing policies, and best practices. Assess and refers clients who require home support or adult day care participation to maintain independent community living. Collaborates, consults and shares information for continuity of care and community resource coordination; and provides education and information services for housing agencies and staff, and other health care providers regarding mental health housing issues. Maintains appropriate client records, participates in in-services and team meetings, and engages in on going program planning and quality improvement activities.
