About The Position

Canadian Mental Health Association Peel Dufferin (CMHA Peel Dufferin) is partnering with various agencies in Dufferin County to offer services at the Dufferin based Homeless and Addictions Recovery Treatment (HART) Hub. HART Hubs are community-based care hubs designed to address mental health, addiction, and housing needs for people experiencing homelessness. They provide integrated care, offering a wide range of services such as primary care, mental health services, addiction care, and employment supports. With targeted community outreach and rapid referral pathways, HART Hubs aim to support individuals with complex needs, helping them recover, achieve housing stability, and inclusion. CMHA Peel Dufferin is an organization dedicated to the mental, physical, and social health of everyone, including its team members. Requirements

  • Post-secondary education in Social Work, Health Services, Psychology or equivalent
  • Minimum 2 years providing Mental Health case management or equivalent care
  • Demonstrated knowledge of serious mental illness, concurrent disorders, and/or dual diagnosis, and experience with assessment
  • Experience working within the homelessness sector
  • Knowledge of evidence-based practice for mental health services
  • Excellent assessment, risk management, and crisis intervention skills
  • Expertise in collaboration with healthcare services
  • Strong interpersonal skills with proven ability to approach, listen, build trust, and communicate across diverse cultures and intersectionality.
  • Willingness to continually improve understanding around cultural competence.
  • Openness to operate from an anti-racist, anti-oppressive, 2SLGBTQ+ affirming and intersectional framework.
  • Willingness to learn and apply practical understanding of different elements of diversity, such as gender identity/expression, race/cultural background, religion, sexual orientation, disability, immigration and precarious status, family status, etc.
  • Valid Canadian driver’s license and access to an insured automobile in good repair
  • A satisfactory Vulnerable Sector Screening (Police Check) report

  • Registered and/or regulated health professional is an asset
  • Fluency in a second language, including ASL (American Sign Language) is considered an asset.

Responsibilities

  • Collaborate in developing and implementing individualized wellness plans with clients
  • Use standardized assessment tools designed to improve and/or stabilize client’s physical and mental health
  • Offer a strength-based approach that recognizes client’s unique needs
  • Provision of case management services including but not limited to Assist with activities of daily living, conflict resolution, and crisis avoidance and mental health counselling
  • Provide comprehensive, holistic, trauma-informed and person/family centered access to services that address the social determinants of health
  • Partner with clients and their families as members of the health care team
  • Develop and maintain a community resource network
  • Refer to partnering organizations and provide linkages to community supports
  • Work independently and collaboratively within a multidisciplinary team
  • Understand and be able to integrate a philosophy of person and family centered care into daily responsibilities
  • Demonstrate a strong commitment to quality care and service delivery based on the HART Hub Mission and Values
  • Ensure appropriate records are kept and ensure that statistical information is maintained as per agency standards
  • Adhere to agency Health & Safety Program and the Client Safety Framework in the promotion of client safety
