About The Position

The Adult Day Care Center located @ Waters Place in the Bronx, is seeking a Mental Health Coordinator to join our team! The Mental Health Coordinator is responsible for the oversight, coordination, and daily operation of the Mental Health Program, ensuring that the clinical, social, and supportive service needs of program participants are effectively met. This position provides direct supervision and guidance to program staff, coordinates case management and care planning activities, and ensures compliance with all Department of Health (DOH) and Office of Mental Health (OMH) regulations and documentation requirements. The Mental Health Coordinator also provides direct social work support to clients, facilitates interdisciplinary collaboration, and communicates regularly with leadership regarding program operations, client needs, staffing, and quality outcomes.

  • Collaboration with transportation staff
  • Reviewing transportation schedules and routes
  • Responding to participant emergencies
  • Serving as a team leader during emergencies
  • Active participation as a member of the interdisciplinary team
  • Developing and maintaining collaborative relationships with community agencies, behavioral health providers, and social service organizations
  • Representing the Mental Health Program at meetings and community events
  • Promoting coordination of care and community resource utilization
  • Attending training programs, seminars, workshops, and continuing education activities
  • Performing other duties and responsibilities related to Adult Day Health Care Program (ADHCP) operations as assigned.

Responsibilities

  • Coordinate all aspects of the Mental Health Program, including intake evaluation and admission of new clients, attendance monitoring, problem resolution, activity evaluation, staff oversight, and staff scheduling.
  • Ensure program operations meet regulatory, organizational, and client-centered standards.
  • Monitor and report program performance, client participation, staffing concerns, and operational issues to the Administrative Manager.
  • Complete initial and ongoing social work and mental health assessments for all clients admitted to and actively participating in the Mental Health Program.
  • Provide ongoing social work support and intervention to clients as needed.
  • Coordinate case management, care coordination, entitlement services, and concrete services for program participants.
  • Ensure all social work and mental health documentation is completed accurately, thoroughly, and in compliance with DOH, OMH, and agency requirements.
  • Conduct comprehensive reviews of client health records prior to discharge to ensure all documentation supports a safe and effective discharge plan.
  • Coordinate, facilitate, lead, and participate in interdisciplinary care planning meetings.
  • Ensure care plans are person-centered, individualized, and coordinated with community healthcare providers.
  • Schedule care plan meetings within required timeframes.
  • Prepare and distribute care plan summaries to Community Case Managers and other appropriate providers.
  • Collaborate with community mental health practitioners and external service providers to ensure continuity of care.
  • Provide direct supervision and oversight of staff assigned to the Mental Health Program.
  • Ensure staff assignments and responsibilities are completed appropriately and timely.
  • Monitor staff performance and report concerns to the Clinical Supervisor and Administrative Manager.
  • Ensure daily documentation is completed accurately and on time.

  • Conduct or coordinate in-service training for staff supporting Mental Health Program participants.
  • Evaluate workloads and motivate staff to achieve program goals and performance expectations.
  • Monitor client attendance and participation according to approved authorizations and schedules.
  • Follow up with Community Case Managers and psychiatric providers to address barriers to attendance and participation.
  • Review daily program activities to ensure adequate staffing and supplies.
  • Identify and report gaps in program delivery to leadership.
  • Ensure monthly activity schedules are developed, posted, distributed to clients, and aligned with participant needs.
  • Collaborate with transportation staff to ensure safe, timely, and effective transportation services.
  • Review transportation schedules and routes to improve efficiency and client access to services.
  • Respond appropriately and promptly to participant emergencies.
  • Serve as a team leader during emergencies and coordinate with appropriate staff, including nursing personnel and other healthcare professionals.
  • Participate as an active member of the interdisciplinary team.
  • Develop and maintain collaborative relationships with community agencies, behavioral health providers, and social service organizations.
  • Represent the Mental Health Program at meetings and community events as needed.
  • Promote coordination of care and community resource utilization for clients.
  • Attend training programs, seminars, workshops, and continuing education activities to maintain professional competence.
  • Perform other duties and responsibilities related to Adult Day Health Care Program (ADHCP) operations as assigned.
