Calgary Foothills Primary Care Network (PCN) is a group of family physicians and health care professionals in northwest Calgary and Cochrane. Calgary Foothills PCN has established a team environment where individuals are encouraged to take initiative, be creative and contribute to ongoing decision-making. The Calgary Foothills PCN Coordinated Attachment clinic supports patients who have complex health or social needs and who may have difficulty connecting with a traditional medical clinic. This team provides wrap around primary care services, and works in a team-based collaborative environment. Practitioners on this team include physicians, a nurse practitioner, a registered nurse, a licensed practical nurse, a social worker, and a mental health clinician. Reporting to the Program Manager, the Coordinated Attachment Mental Health Consultant collaborates with the team and provides mental health supports for the patients attached to this clinic.
