They offer evidence-based treatment for adults, youth, and seniors facing behavioral health challenges, operating as a Trauma-Informed Care Organization that believes in client recovery and meaningful lives. The multi-disciplinary Homeless Outreach Stabilization Team (HOST) works in the community to provide substance abuse treatment, medical services, rehabilitative support, case management, and peer support. This program specifically targets homeless individuals with behavioral health challenges who have continuous service needs and functional impairments and have not benefited from traditional outpatient programs. The Mental Health Clinician II will collaborate with a multi-disciplinary team to serve homeless adults with severe and persistent mental illness and substance use disorders, providing targeted services to highly vulnerable individuals lacking service connections or support and unable to engage in traditional models. The role involves collaboration with the medical team, providing psychoeducation, and conducting clinical case reviews. Services are delivered in various community settings such as emergency and transitional homeless shelters, encampments, transportation hubs, and other public gathering places. The position requires knowledge of service delivery models for adults with serious and persistent mental illness, effective communication with diverse populations, performing mental health assessments and developing treatment plans, facilitating groups, providing training on substance abuse, advocating for client access to resources, and participating in team meetings and trainings. FBH charting, correspondence, and paperwork must be completed in a timely manner, adhering to clinical accountability policies and maintaining complete clinical records. Travel to community locations using FBH vehicles or personal vehicles is required.
