POSITON TITLE: Mental Health Case Manager Non-Exempt Position RESPONSIBLE TO: Program Director MAJOR DUTIES AND CORE COMPETENCIES Under the supervision of the Program Director and Program Supervisor, the Mental Health Case Manager provides community-based behavioral health, care coordination, and re-entry support services to participants. The position requires independent judgment, effective collaboration, regulatory compliance, and a commitment to recovery-oriented, trauma-informed, and person-centered care.

Requirements

  • Bachelor's degree in Behavioral Health, Psychology, Social Work, Human Services, Counseling, or a closely related field preferred.
  • Washington State Department of Health credential required or ability to obtain and maintain required credentialing.
  • Minimum of one year of paid or volunteer experience working with individuals experiencing mental health, substance use, co-occurring disorders, or related behavioral health conditions.
  • Working knowledge of behavioral health recovery principles, trauma-informed care, and participant-centered service delivery.
  • Familiarity with applicable Washington State behavioral health regulations, including RCW 71.05 and WAC 246-341.

Responsibilities

  • Conduct comprehensive assessments, including whole-person and Health-Related Social Needs (HRSN) assessments, to identify participant strengths, barriers, risks, and service needs.
  • Develop, implement, and monitor individualized Re-Entry Care Plans that are strength-based, participant-centered, culturally responsive, and aligned with identified goals and service needs.
  • Regularly evaluate participant progress and adjust interventions and service plans to support successful community integration and recovery outcomes.
  • Provide targeted re-entry case management services in accordance with Washington State's Medicaid Re-Entry Demonstration Initiative (1115 Waiver), agency standards, and clinical recommendations.
  • Coordinate access to behavioral health treatment, substance use disorder services, medical care, housing resources, transportation, employment supports, benefits, and other community services.
  • Facilitate incoming and outgoing referrals and ensure continuity of care across providers, agencies, and systems.
  • Utilize sound judgment and problem-solving skills to address participant needs, barriers to care, and service engagement challenges.
  • Serve as an active member of the multidisciplinary treatment team by contributing clinical observations, recommendations, and participant progress updates.
  • Collaborate effectively with correctional facilities, healthcare providers, community agencies, managed care organizations, probation officers, courts, and other stakeholders to support participant outcomes.
  • Maintain timely, accurate, and complete documentation of all services, contacts, assessments, and care coordination activities in accordance with: WAC 246-341, RCW 71.05, Medicaid Re-Entry Demonstration requirements, HIPAA and 42 CFR Part 2 confidentiality requirements, Agency policies and procedures.
  • Demonstrate accountability for compliance with federal, state, and agency regulations governing behavioral health service delivery.
  • Support agency quality improvement activities, accreditation standards, and performance outcomes through accurate documentation and adherence to best practices.
  • Develop and maintain collaborative relationships with community agencies, service providers, and referral partners to improve participant access to resources and support services.
  • Identify community resources and advocate for participants to obtain services necessary to support recovery, stability, and successful re-entry.
  • Represent the agency professionally in community meetings, care coordination activities, case staffing, and other approved forums.
  • Demonstrate professionalism, ethical decision-making, cultural humility, and adherence to agency values and professional standards.
  • Participate in supervision, training, staff development, and continuous quality improvement activities.
  • Support and implement agency policies, procedures, and organizational initiatives.
